Aden Duale, the Health Cabinet Secretary, and Kamel Ghribi, the chairman of the GKSD Group, signed the contract in front of President William Ruto at State House Nairobi.
Fifteen hospitals throughout 15 counties are covered by the program, which is being developed under Italy's Mattei Plan for Africa.
Two Level 6 national training and referral facilities and thirteen Level 5 county referral hospitals form the package.
The estimated value of the investment is approximately Sh54 billion, or US$419 million.
It is a Letter of Agreement and healthcare partnership that establishes the parameters for project development and funding.
It does not indicate that Kenya has spent or received the whole US$419 million.
The Kenyan projects will continue to go through the finance and execution phases of Italy's larger Mattei Plan, which employs a combination of public and private funding.
Mombasa and Uasin Gishu will host the two Level 6 hospitals.
The planned Mombasa National Teaching and Referral Hospital at Jomvu is the initiative in Mombasa.
The President designated Moi Teaching and Referral Hospital in Eldoret as the additional Level 6 facility in Uasin Gishu.
The Eldoret project is not a brand-new initiative that emerged from Tuesday's agreement because it is already under construction.
In addition to the remaining counties in the 13-facility program, the President designated Kilifi, Mandera, Marsabit, Embu, Migori, Nyamira, Turkana, Kisii, Baringo, Bomet, Narok and Murang'a as the counties selected for the Level 5 hospitals.
The President's most recent declaration identified Kisii and Murang'a, while the Ministry of Health named Nakuru and Laikipia for the 13 planned Level 5 hospitals.
